Abstract

Uterine leiomyoma is one of the most common benign tumors in women of reproductive age. Cystic degeneration is a rare form of leiomyoma degeneration that may create diagnostic challenges, particularly in patients with primary infertility. This condition may mimic molar pregnancy or gynecological malignancy on imaging examinations, increasing the risk of misdiagnosis. We report a case of a 33-year-old woman presenting with an abdominal mass and primary infertility who was initially suspected of having a molar pregnancy based on ultrasonographic findings. Ultrasonography revealed an intrauterine cystic mass with multiple hypoechoic areas and a honeycomb appearance. Urine pregnancy test results were negative, and serum β-hCG levels were <2.00 mIU/mL. Curettage followed by histopathological examination confirmed the diagnosis of uterine leiomyoma with cystic degeneration. This case highlights the importance of comprehensive evaluation in diagnosing uterine masses in infertility patients, including imaging studies, laboratory examination, and histopathology. Accurate diagnosis is essential to determine optimal management and preserve fertility potential. A multidisciplinary approach is important to reduce the risk of misdiagnosis and improve clinical outcomes.
