Coronary artery disease is one of the leading causes of morbidity and mortality in the elderly population. The increasing prevalence with age poses complex clinical challenges, especially due to the unique characteristics of heterogeneous elderly groups. The differences between chronological and biological ages demand a more specific assessment in determining diagnostic and therapeutic strategies. In addition, factors of physical and social functioning, the presence of geriatric syndrome such as cognitive weakness or decline, and the frequent comorbidities that accompany it, must be considered comprehensively. Standard approaches to cardiology are often inadequate to address this complexity, so multidisciplinary evaluations involving geriatric aspects are required. This study aims to summarize the current conditions regarding the diagnosis and management of acute coronary syndrome in elderly patients, emphasizing the importance of individualization of therapy based on functional status, cognitive capacity, and comorbidity burden. Thus, clinical decision-making in this subgroup not only focused on cardiological parameters, but also considered quality of life and long-term care goals.
